What is TMG's Bereavement Policy for the death of immediate and non-immediate family members?
Associates are allowed 3 days of paid bereavement leave in the event of the death of an immediate family member (spouse, child, mother, mother-in-law, father, father-in-law, sister, brother, stepmother, stepfather, stepsister, stepbrother, stepdaughter, stepson). Associates are allowed 1 day of paid bereavement leave in the event of the death of a non-immediate family member (sister-in-law, brother-in-law, daughter-in-law, son-in-law, aunt, uncle, grandparent, grandchild, spouse’s grandparent). Am I entitled to Bereavement Leave if another associate passes away?
Associates are allowed up to 4 hours of paid bereavement leave to attend the funeral of an associate or retiree of the company. |
Direct Deposits |
How do I set up a Direct Deposit?
To set up a Direct Deposit account for your pay checks, you will need to complete the Direct Deposit form (found HERE) and submit a voided check or letter from your bank that includes the routing and account number associated with your account. Your Direct Deposit form must match the information on the additional documentation and cannot be handwritten. Do I need to submit a Direct Deposit?
No, TMG does not require associates to submit a Direct Deposit form. However, we no longer provide our associates with paper paychecks, so you will receive a "paycard" if you do not submit adequate Direct Deposit information. What is a paycard?
A paycard takes the place of paper paychecks and works similar to a credit/debit card. Associates that do not submit Direct Deposit information and/or do not have bank accounts will receive a paycard to receive their pay electronically. Who do I contact if I lose my paycard?
For lost or stolen cards, associates should call Global Cash Card via phone at (888) 220-4477 and contact the Payroll department to request a replacement card. Are there fees associates with using a paycard?

What is FMLA?

An associate is eligible for FMLA if he or she has been employed for at least 1 year, or has worked at least 1,250 hours. The employee must work at a location that has 50 or more employees within a 75 mile radius. An employee may request FMLA for the following reasons:
What qualifies as a serious health condition?
A serious health condition is defined as a condition that incapacities you or a family member causing the person to be unable to work or go to school for over three consecutive days, and seek ongoing medical treatment. Pregnancy also qualifies as a serious health condition. Am I required to use my accrued time?
Yes, the employer may request to use all accrued time if the employee is eligible for FMLA. We require FMLA run concurrently with any accrued time. Do I have to take leave all at once or can it be taken periodically?

When am I eligible for holiday pay?
All full-time associates of TMG are eligible for holiday pay after 90 days of continuous employment. Associates are also required to work (or have pre-approved vacation) on the business day before and after a holiday in order to be paid. How do I make sure I am paid for the holiday?

Who do I contact if I would like to transfer to another location within The Millennium Group?
If you are interested in transferring to one of our other accounts, you will need to first inform your manager and formally apply for the position on our website. You should then contact TMG's Talent Acquisition team via email at [email protected] with your interest. How do I know if I am eligible for a transfer?
To be considered for a transfer, associates must have a satisfactory performance record and have longevity in their current position. A review of disciplinary warnings will be considered. You will also need to pass the background check requirements for the transfer location. Are there any exceptions to this policy?
Yes, management retains the right to make exceptions to this policy to meet the needs of the company. Exceptions are typically considered for promotions, temporary projects, and lay offs. |
JURY DUTY |
What do I do if I receive a court summons for Jury Duty?
Upon receipt of notification from the state or federal courts of an obligation to serve on a jury, associates must notify their supervisor and provide him/her with a copy of the jury summons for scheduling purposes. Do I get paid for Jury Duty? Yes, TMG will pay associates required to serve jury duty $40.00 per day for a maximum of three (3) days OR their regular pay as governed by state specific laws. Associates are required to provide HR and/or the Payroll Department a copy of their Attendance Record from the court for Jury Service in order to be paid for the time away. |

Short Term Disability (STD) is insurance that pays a weekly benefit to the disabled employee in any event that he or she may not be able to work due to a non-work related illness or injury. STD is a cash-benefit and replaces a portion of the employee’s pay. When am I eligible for disability?

An employee is eligible to receive disability payments after the eighth consecutive day of being out of work and not receiving any sort of pay
How do I begin the disability claim process?
Contact Human Resources to obtain a short term disability claim form. The claim packet will contain an Employee’s Statement, a Physician’s Statement, an Employer’s Statement, and a HIPPA authorization. Please complete the Employee portion and send the claim form to your treating physician to complete. Please submit the completed claim documents to your HR representative to complete the Employer portion and HR will submit the claim form directly to The Millennium Group’s disability insurance company. How long will it normally take for a claim decision?
Once the short term disability claim forms are completed entirely and submitted to the insurance company, it will take approximately one week for a claim decision to be made. If a decision is not made within one week, you will be notified with details or sent a request for further information. What if I need to take some time off of work?
The employee may apply for FMLA (Family and Medical Leave Act). Disability runs concurrently with FMLA. An employee is eligible for FMLA if he or she has been employed for at least 12 months or has worked 1,250 hours. FMLA provides the disabled employee for up to 12 weeks of unpaid job protection if he or she has a qualifying serious health condition. When do STD benefits end?
